LEEP Forward, Inc. is an In-Network provider with BlueCross BlueShield PPO and BlueCross BlueShield Blue Choice only.  LEEP Forward will submit claims directly for families who have a Blue Cross Blue Shield plan. For those with other insurance carriers, we will create invoices for families to send in. We will provide supporting documents as needed.
